SideStix are a non-standard code item. Therefore, you are required to provide the following documentation in one of two ways outlined below. Please call us if you have questions!

With support from your Occupational Therapist or Physiotherapist:

The following will need to be provided:

    1. SideStix Justification Letter
    2. SideStix Client Functional Assessment and Justification Form from your occupational or physiotherapist.
-Sample Forms can be seen here: Joint Replacement, Amputation, MS
    1. SideStix Quote or a receipt from the purchase of SideStix.
-To get a Quote run through the shopping cart process on the website, choose the desired equipment and click Quote at the end.

OR

With support from your Doctor:
The following will need to be provided:

    1. The SideStix Justification Letter
    2. Physician’s Prescription stating that SideStix “will maintain and / or enhance your everyday life skills in and around your home”, based on the justification letter.
    3. SideStix Quote or a receipt from the purchase of SideStix.
-To get a Quote run through the shopping cart process on the website, choose the desired equipment and click Quote at the end.

General VA Inquiries:

-VAC Health Identification Cards accepted.

Ministry Coverage: Workers Compensation, Worksafe, MSP (Under 18 years old ONLY), etc.

-Please complete the same documentation process as outlined above and contact your local medical equipment store.
